The Product Decision Gap: Why What You Decide and What Gets Built Are Never the Same Thing

There is a moment in every sprint that no one talks about openly. The PM wrote a ticket. The engineer read it. And somewhere in that translation, the product that gets built is subtly — sometimes not so subtly — different from the product that was decided.

Not because anyone made a mistake. Not because the spec was bad. Just because decisions are made in context, and by the time they reach code, most of that context is gone.

This is the product decision gap. And it is one of the most expensive problems in software that almost no team has a name for.

What Actually Happens When You Make a Product Decision

Let’s trace a fairly ordinary decision. A PM is in a meeting with design, engineering, and a customer success lead. The customer has been asking for bulk actions in the dashboard. There is a conversation — maybe 40 minutes — about which actions make sense, what the edge cases are, why certain things should require confirmation, what undo behavior should look like. There is a whiteboard. There are opinions. Someone brings up a previous incident where a user accidentally deleted everything.

At the end of the meeting, there is consensus. A decision has been made. Everyone leaves with roughly the same mental model.

Then the PM writes a ticket.

Even a thorough ticket — five paragraphs, a Figma link, acceptance criteria — captures maybe 30% of what was in that room. The edge case discussion? Summarized in one line. The reason certain actions require confirmation? Not in the ticket. The previous incident that shaped the whole conversation? Nowhere.

The engineer who picks it up three days later, in a different headspace, working on a different mental model of the codebase, has to reconstruct intent from that 30%. They make reasonable assumptions. They ship something. It is good work. But it is not quite what was decided.

Three Places Where Fidelity Dies

The decision gap is not a single failure — it is a sequence of compressions, each one losing a little more of the original signal.

The meeting-to-spec compression. This is the biggest one. Spoken reasoning — the “why,” the alternatives considered, the emotional weight given to certain concerns — almost never makes it into written specs. Tickets document what. They rarely document why, and almost never document what we decided not to do and why. That negative space is where a huge amount of product wisdom lives.

The spec-to-ticket compression. A spec is usually written in prose. A ticket is structured for execution. In that translation, nuance gets flattened into acceptance criteria. “Should feel lightweight and not interrupt the user’s flow” becomes a checkbox: “Modal confirmation for bulk delete.” The spirit is gone. The letter remains.

The ticket-to-implementation compression. Engineers make dozens of micro-decisions while building that are never surfaced back up. The interaction state that was ambiguous in the spec? They picked one. The edge case that wasn’t covered? They handled it in a way that felt reasonable. Most of these decisions are invisible. They compound.

Why This Is Getting More Expensive

Teams have lived with this gap for decades. It is not new. But three things have made it significantly more costly in the last few years.

Software complexity is compounding. A modern product has more surface area, more integrations, more states, more edge cases than products from five years ago. The more complex the system, the more the gap matters. A small misread on a simple feature is a one-hour fix. A small misread on a feature that touches authentication, billing, and three third-party APIs is a two-week rework.

Teams are more distributed. Synchronous context transfer — the hallway conversation, the shoulder-tap, the five-minute standup sidebar — used to paper over a lot of the gap. When your PM is in London, your engineer is in Bangalore, and your designer is in São Paulo, you lose those informal context channels. Written artifacts are doing more work than they were designed to do.

AI-assisted development is accelerating the build loop. This is the new one. When engineers can ship features in a quarter of the time, the decision gap gets hit more often, faster. The velocity is real. But if the signal fidelity going into the build has not improved, you are just building the wrong thing faster. Speed amplifies mistakes as readily as it amplifies good judgment.

What High-Fidelity Product Teams Actually Do

The teams that handle this best are not doing anything magic. But they are doing a few specific things differently.

They write decision documents, not just specs. There is a meaningful distinction. A spec describes the output. A decision document describes the reasoning: what we considered, what we ruled out, what trade-offs we accepted, what would make us revisit this. It is more work to write. It is dramatically cheaper than re-litigating the decision six weeks later when the build does not match the intent.

Some teams use a lightweight ADR (Architecture Decision Record) format for engineering decisions and a similar “PRD + rationale” format for product decisions. The format matters less than the habit. Write the why. Write the not-this.

They distinguish between reversible and irreversible decisions. Jeff Bezos’ famous Type 1 / Type 2 decision framework gets referenced a lot, but fewer teams actually operationalize it at the ticket level. High-fidelity teams tag decisions by reversibility. Reversible decisions get lighter documentation, faster execution, easier rollback plans. Irreversible ones — data model changes, public API shapes, pricing structures — get reviewed differently, documented more thoroughly, and built with more checkpoints.

They close the implementation feedback loop. One of the most underrated practices: require engineers to write a brief “what I built and why I made these calls” note when they close a ticket. Not a changelog — a reasoning log. What was ambiguous? What did you decide? What would you flag for a product conversation?

This closes the gap from the other direction. Instead of just trying to push more context downstream into the build, you are also pulling context upstream from the build back into the product record.

They make the negative space visible. The decisions not made, the features not built, the alternatives not chosen — this is where teams keep stepping on the same landmines. A running “considered and rejected” log, even just a section at the bottom of a Notion page, prevents the wheel from being reinvented in every quarterly planning cycle. “Why don’t we just add a CSV export?” Because we considered it in Q3 and here is why it doesn’t work with our data model.

The AI Question

A reasonable thing to ask at this point: can AI close the gap?

Partly. There are genuinely useful applications here. Meeting transcription and summarization can capture more of the spoken reasoning that used to die on whiteboards. AI-assisted spec writing can prompt for the sections that humans tend to skip — edge cases, reversibility, what success looks like. Some teams are experimenting with AI that watches a PR and flags when the implementation seems to diverge from the stated intent in the ticket.

But the core problem is not a tooling problem. It is a culture-of-documentation problem dressed up as a tooling problem. No AI tool will save a team that does not value the written articulation of reasoning. It will just make the gap faster to fall into.

The teams getting actual leverage from AI in their product process are the ones who already had decent documentation hygiene. AI makes good processes faster. It does not make bad processes good.

A Practical Starting Point

If this resonates, and you want to actually move on it rather than just nod along, here is the smallest intervention that has a disproportionate impact:

For the next three major features your team ships, require one additional field in the ticket: “What would make us revisit this decision?”

That is it. One field. But filling it out forces the writer to be explicit about the assumptions baked into the decision. It surfaces the conditions under which the decision might be wrong. It gives future-you a trigger for re-evaluation that is not just “it felt off.”

Over time, this single habit starts pulling the rest of the documentation culture forward. If you know you need to articulate what would change this decision, you start thinking more carefully about the reasoning behind it. You write it down. Others read it. The gap gets a little smaller.

The Long Game

Product quality is not just about the quality of your decisions. It is about how much of your decision-making intelligence survives the journey from the conversation to the code.

The teams building software that feels considered — where the edges are handled, where the experience is coherent, where it is obvious someone thought hard about this — are not necessarily making better decisions in the room. They are doing a better job of preserving those decisions all the way through to the thing that ships.

That is a learnable skill. It is not glamorous. It does not have a good demo. But it is one of the highest-leverage investments an engineering org can make, and almost no one is talking about it.
